The short answer
Botox aftercare is simple, but the first few hours matter most. For roughly the first 4 hours, stay upright and avoid touching the treated area; skip intense exercise, heat and alcohol for 24–48 hours. Do that, and your results settle smoothly over the next two weeks. Here's exactly what to expect, day by day.
Four rules for the first few hours
If you remember nothing else, remember these. They cover the short window right after your appointment.
-
Hours 0–4
Stay upright
Keep your head up — sit, stand or walk. Avoid lying flat.
-
Same day
Don't rub
No rubbing, massaging or pressing the treated area.
-
24–48 hrs
No heat
Skip saunas, hot showers and anything that flushes the face.
-
24–48 hrs
No workout
Pause intense exercise; light walking is fine.
At a glance
The quick version
- Stay upright ~4 hours after your appointment.
- Don't rub, massage or press the treated area.
- Skip intense exercise for 24–48 hours.
- Avoid heat — saunas, steam rooms, hot yoga, sunbeds, very hot showers — for 24–48 hours.
- Hold off alcohol for the first 24 hours.
- Gentle facial expressions are fine — no need to keep your face still.
Note: Always follow the specific instructions your injector gives you — they may adjust this based on your treatment.
What to expect, day by day
A neuromodulator works gradually, so your result builds over about two weeks rather than appearing all at once. Here's the typical arc.
A typical timeline. Your own pace may differ — individual results vary.
Day 0 — the day of your treatment
Tiny bumps or slight redness at the injection points are normal and usually fade within a few hours. You can return to most of your day right away — just keep it calm: stay upright, leave the area alone, and skip the gym and that glass of wine.
Days 1–2 — settling in
Any pinpoint marks have usually faded. You might notice mild tenderness or, occasionally, a small bruise. Most people are back to their full routine — exercise, facials and heat can wait until the 48-hour mark to be safe.
Days 3–4 — it starts working
This is when many people begin to notice the area feeling smoother as the neuromodulator takes effect. The change is gradual, not instant.
Days 7–10 — most of the effect
By now the result is well underway. If something looks slightly uneven, don't worry yet — it's still settling.
Day 14 — the final result
Botox typically reaches its full effect around two weeks. This is the right time for any complimentary review with your injector, who can assess whether a small adjustment would help.

What's normal — and when to check in
Most of the early window is uneventful. This is the simple line between "expected" and "worth a quick call".
-
Normal — settles on its own
- Pinpoint redness
- Tiny bumps for a few hours
- Mild tenderness
- A small bruise
- A slightly heavy feeling as it kicks in
-
Worth contacting your provider
- A drooping eyelid or brow
- Marked asymmetry that doesn't settle
- Signs of infection — spreading redness, warmth, pus
- Trouble swallowing or breathing
- Any reaction that feels wrong to you
When in doubt, reach out — your injector would always rather hear from you.

Everyday questions
The little things people ask about
Makeup
Wait about 4 hours before applying makeup, and apply it gently — no pressing or heavy rubbing over the treated area.
Sleeping
For the first night, try to sleep on your back if you can, and avoid pressing your face into the pillow. For more detail, see our guide on how long after Botox you can lay down.
Exercise
Light walking is fine the same day. Save intense workouts, hot yoga and anything that gets your heart rate up for 24–48 hours.
Skincare & facials
Keep your routine gentle for a day or two. Postpone exfoliation, retinoids if your injector advises, and any heat- or laser-based facials.
Flying & sun
Short flights are generally fine. Be sensible with sun and heat for the first couple of days.
